At Celine Health, we are dedicated to improving the well-being of vulnerable community members through compassionate, personalized care. Based in San Diego, we specialize in enhanced care management and community support, addressing both clinical and non-clinical needs. By fostering strong connections and coordinating health services, we empower individuals to navigate their health journeys with dignity and resilience, working toward a healthier, more inclusive community for all.

Enhanced Care Management (ECM) Is a personalized program that helps individuals with serious health conditions receive coordinated care at no cost for those with a managed care plan. ECM supports creating care plans, ensures proper treatments, provides health education, and connects individuals with community services. The program works with doctors, health plans, and local organizations to deliver the best care.

Community Support consists of 14 programs designed to improve members' health and well-being by addressing social needs and preventing more costly care. Eligible members may receive services if deemed medically appropriate and cost-effective by the Managed Care Plan (MCP).
